Announcing a new article publication for Cardiovascular Innovations and Applications journal. This article reports a successful rescue technique for valve migration during transcatheter aortic valve replacement (TAVR) in a high-risk patient with pure aortic regurgitation, a condition that poses major anchoring challenges due to the lack of calcification. In this case, downward migration of the initially implanted valve caused paravalvular leak and impaired mitral valve function. The authors introduce a novel “second valve drag and reposition” (SVDR) technique, using a second transcatheter valve to safely pull the migrated valve upward and achieve stable valve-in-valve implantation. The procedure restored normal valve and mitral function with no complications, and follow-up at one year confirmed durable results, highlighting SVDR as a promising bailout strategy when conventional repositioning methods fail.
